4feb575e3a #19684: Fixing cartesian products query when loading content type definitions 
Using FetchMany on multiple levels would do LEFT JOIN, creating an exponential
amount of rows, and also duplicating root records. The solution is to separate
this is two calls instead. Another solution would have been to use a
DistinctRootTransformer, but this would not have fixed the records issue.

dbe908a717 Exception handling consolidation in command handlers. 
Use Context.Output to write error messages when you handle an Exception or an error case. If you can't recover afterwards, just return.
Don't throw an exception from within the handler unless necessary.
-> If you want to throw due to an inner exception, throw an OrchardException. Add an error message and the exception as the innerexception of your OrchardException.
Your exception will be caught by the command executive, an error message will indicate which command failed, and the inner exceptions will be displayed.
Also, do not return values from commands, they are not services.

db3046f68f Fix 1.0 to 1.1 migration issue 
SiteSettings are used very early and often in Orchard, so we
can't rely on having the user be able to run migration for
SiteSettings. The workaround for 1.1 is to move the changes
we did to SiteSettings into a new SiteSettings2 Part/Record.

With this change, database compatiblity from 1.0 and 1.1 seems
to be in a good working state on a real blog site.
